A ParQ (Physical Activity Readiness Questionnaire) is a short health questionnaire that new members complete when they join your club. It helps you identify anyone who might need extra support or medical clearance before they start training. ClubRight lets you build your own ParQ from scratch or from templates, and flag specific answers as high-risk so your team sees an alert when those members check in.
⚠️ Important: If your ParQ has questions set up, it is live. There is no on/off switch. Members joining online will be asked to complete it as part of their sign-up.
Where to find ParQ Setup
Open Settings from the burger menu, expand People & Access in the left sidebar, and select Health questionnaire (ParQ). The ParQ Setup screen opens with a Your questions panel and a How does this work? guide alongside it.
How to add a question
Click + Add another question.
Type your question in the question field, for example "Do you have any heart conditions?".
Enter up to three answer options. The first two ("Yes" and "No") work for most ParQ questions, with an optional third option if needed.
For each answer, decide whether to enable:
Ask for details: when a member selects this answer, they will see a text box to explain further. Use this when you need more information, for example if someone answers "Yes" to having a medical condition.
Flag as high-risk: tick this on any answer that should flag the member as high-risk, for example "Yes" to chest pain. Staff see a HIGH RISK PAR-Q ALERT when that member checks in.
Click Save Changes in the top right.
Reordering and deleting questions
To reorder questions, use the grip handle on the left of each question to drag it into place. To remove a question, click the bin icon on the right of the question row. Click Save Changes in the top right to confirm.
Using Scenario Templates
If you would rather start from a ready-made set of questions, click Scenario Templates at the top right of the ParQ Setup screen and choose a template that matches your club. You can edit any of the loaded questions to suit your needs before saving.
What members see when joining
When a new member joins online, the ParQ appears after they choose their membership plan and before the payment screen. They complete the questions, and if any answer has Ask for details enabled, an extra text box appears for that question. Once submitted, their answers are saved against their member profile.
How to review a member's ParQ
Open Contacts from the burger menu and find the member.
Open their full profile.
Click the ParQ tab to see their answers, including any extra detail they have provided.
If a member did not complete their ParQ online (for example, they joined in-club), you can add it from their profile and ask them to complete it then and there.
What happens when a high-risk member checks in
If a member's ParQ answer was flagged as high-risk, your staff see a HIGH RISK PAR-Q ALERT when that member checks in. This is a visible prompt for your team to review the member's ParQ before proceeding, so they can apply any precautions needed.
Helpful tip
💡 Less is more. Keep your ParQ short and to the point. A long list of questions can put new members off during the sign-up flow. Five well-chosen questions cover most clubs and stay easy to complete on mobile.
Example questions to start with
If you are not sure what to include, the following questions are a good starting point. Use Flag as high-risk on the answers that should trigger a check-in alert, and Ask for details where you want a member to expand on their answer.
Do you have any heart conditions?
Do you experience chest pain during physical activity?
Do you have high or low blood pressure?
Are you pregnant or have you given birth in the last 6 months?
Are you taking any medication we should be aware of?
Is there anything else we should know about your health or medical background?
